Researchers at the University of California - Davis Health have challenged a long-held assumption: that reaching 90 without dementia is a reliable indicator of good health. A study published in The Lancet Healthy Longevity reveals that even among individuals who have reached their tenth decade, certain groups remain disproportionately affected by dementia.

The LifeAfter90 study, which has been tracking adults over 90 since 2018, sheds light on the persistence of dementia disparities across different demographics. Women in this age group face approximately twice the risk of men, echoing previous findings among younger populations. The research also highlights striking racial and ethnic differences, with black participants exhibiting a 75% higher risk than Asian participants.

According to Rachel Whitmer, professor of public health sciences and neurology at UC Davis Health, healthcare professionals often rely too heavily on age as a proxy for health status. “We can’t just assume that someone from a high-risk group makes it to 90 without dementia and they’re in the clear,” she notes.

The study’s focus on the APOE gene, long associated with Alzheimer’s disease, reveals complex interactions between genetics and age. While the protective effect of APOE2 remains strong even beyond age 90, APOE4 produces a more nuanced pattern. In the overall population, this variant does not substantially increase dementia incidence; however, specific groups exhibit increased risk.

Among black participants carrying APOE4, for example, dementia risk approximately doubles. This highlights the need for clinicians to consider individual genetic profiles when assessing patients’ risk of cognitive decline. As Whitmer suggests, “Doctors need to know that certain groups are at higher or lower risk.”

The study also raises an intriguing question: what enables some individuals to remain cognitively healthy despite carrying significant dementia risk factors? Researchers now seek to uncover the biological and environmental mechanisms responsible for this phenomenon, potentially leading to new avenues for preventing or mitigating dementia.
